I would do the following things:

1) I would author a defect report that describes the issue in detail and assign it a correspondingly high Priority.

2) I would inform all invested and concerned stakeholders (Project Manager, Test Management, relevant members of business sector).

3) If the showstopping issue was found in production, I would initiate a process to quickly recover from it.
 
4) If the issue was discovered during a release build, I would evaluate our release options in a meeting with expert and concerned stakeholders.
  
5) Depending on how late in the testing phase this issue was found, I would attempt to discover how we missed the issue in previous test cycles.
