Missing Requirements after SRS Sign off

After SRS sign off, how do you take the missing requirements and in which document you place them if taken. What is the document maintained for change request?

Usually the new requirements are considered as change requests if they are to be worked on in the same phase. There could be a CR tracking sheet which will have all such requests.
Otherwise they would be part of next phase SRS.
