after releasing of first version of software , when user need some changes in previous verion , then its called "change request"

A change request mainly involves changes from client requirement, resource, QMS document.

In a project, before the development could start the client requirements are freezed.

When a client requests for a new change to the freezed requirements it is considered a Change Request from client, which would be authorized by the fucntional head having the time line for the new requirements. The Client approves/disapproves the time line.

Each Client Change request are numbered as CMR01, CMR02...

A change request serves to raise attention to a recommended modification to a program, process, or system.  It indicates that a need exists but does not describe how this need will be fulfilled.  They commonly originate from enhancement requests submitted by end-users, but they can also be submitted by any company stakeholder who identifies a necessary change.

Change requests typically include the following information:
1) Title
2) Date
3) ID
4) Priority
5) Originator (or requestor)
6) Deadline (target build, date, etc)
7) Description (or abstract)
