For long term projects we need a complete business requirement specification (BRS)document, Customer Requirement specification document, design document for testing, Test plan, Test strategy document, project plan.
But for small project we don't need all these documents, because we can perform adhoc testing.

the most important would be an SRS since for testing the complete stgudy has to be done hence SRS is most important. apart from this design documents, test plans, test suits are required

QA: It needs might be differing from company to company depending upon the CMM level or the certification which company might be using,
Testing: It will definitely will include the SRS and the Test Plan as it clearly brings about the scope, dependencies etc which are to required while Testig a particular application.

Documents for QA,QC/ Testing depends upon company to company standards and procedures to carry on the process.Generally,Project Plan,SRS,BRS,Design plan,use cases are reqd to prepare Test Plan for QA/Testing,test scenerios,test strategy,test cases,tracebility matirx,acceptance test plan,all types of testing phases' test plan if reqd.,checklists.Besides that,we will be having test result document,bug report document,test matrics documents,etc,The template for all the documents will vary according to company standards.

The answer depends on the industry, company, and project. In some cases, only the Business Requirements Document (BRD) is needed. In other cases, any combination of Technical Specifications, Work-Flow Diagrams, and Wireframes are also helpful (if not required).
