Bug report contains complete description of the bug such as Identifier, Description, Status of the defect, Severity, Priority, Platform, Product Name, Product Version, Detected during (development, certification etc), Detected by, Detected on Date, Frequency, Whether the defect is Reproducable or not.

You would also include specific steps to recreate the problem, e.g., starting state, actions performed, etc., often in a detailed description field.

Bug report should contain bug ID,summary about the bug,reproducible steps,detailed description about the bug,status,severity,functionality being tested,version which u r testing,responsible person.

After we find the bug then put it in BTS . the BTS s are diff types.

My company is used the fields as

 --> ProjectName, ModuleName,Screen Name

 --> DeveloperName, Source Name (Design, Development tc),Source   

       Type

 -->Priority, Impacts (Alpha, Beta, Functional etc)

--> Version No, Severity, Testlevel, Testcase Name

--> Cause, Cause Category Type

--> Bug summery,Expected, Steps to reproduce  with status of the bug (New, Reopen )

Basic elements of defect report are
Defect ID,
Summary,
Description (steps to reproduce, environments, Expected values, Actual outcome),

Status,
Severity,
Priority,
Detected by,
Detected on,
Module, build/version id,
Attachments (screen shots or docs),
Reproducible (y/n).

The basic elements can include:
1. Issue Description
2. Pre-Requisite or Data Setup (whichever suitable)
3. Steps to reproduce
4. Expected Result
5. Actual Result
6. Any cause observed for the occurence of bug
7. The effect observed due to the cause

(6th and 7th can be optional when we speak about the basic elements)
