You can follow these points and cover them based on your actual project.

1. Name of the Project
2. Name of the Client

3. Brief Description of the Client
4. Brief Description of the Project
5. Purpose of the project.
6. Specify the front end and back end tool and technology used in the project
7. Brief Description of Features and Reports developed and its significance
