m means the number of mix & 5 refers to the strength of concrete block of size 150x150x150 mm after curing of 28days.

Important term in the question is "Design mix".

You see there is a term "Nominal mix" in concrete which is pertaining to certain standards and thumbrules with respect to properties of concrete materials such as aggregate, sand, cement, water and admixtures.
 
However in order to get the most economic/workable mix of the concrete, it is designed and tested in different proportions by having taken care of exact physical & chemical properties of the ingredients.That is called the design mix of any grade of concrete.

Now normally M5 is used in PCC and hence design mix is not done for the same but for grades M15 and above.

m means grade of concrete refers to the strength of concrete block of size 150x150x150 mm after curing of 28days. which is called as M5 M=mega pascal = N/mm2  or M5=5 N/mm2

Design mix for M5 is 1:5:10 (1-cement, 5-sand, 10-Coarse aggregates)
