This varies case to case in some orgnizations PMP will it self have a test plan in this PM and QA M/L are responcible to prepare this document. Eventually it is primary responcibilty of the QA manager to prepare Test plan.

It completely depends on the organisation you are working in.

In some organisation Test Manager prepares test plan and in some organisation Test Lead prepares the test plan.

In some organisation entire Test Team have a meeting in presence of the Test Lead and they together decide on the test plan.
