Traceability matrix is used to find out how many of the customer requirements are covered how many of requirement not covered in each and every phase of SDLC.

For that at initial stage we required documents mainly BRS/SRS(Business Requirements Specifications/Software Requirements Specifications). To know the coverage of requirements we need other documents also regarding SDLC phase.

Depends on the companies the format and SDLC phases will be change.

To know the coverage of requirements we need the each and every document which are using for application development.

Generally some numbers or names are allocated for the documents in each and every phase for simplicity and to mention in the traceability matrix .
