There are 3 reasons for the occurence of this situvation

1. The defect has occured due to the Miss-interpretation of client. In this phase we have to provide the complete actual functional flow once.

2. Problem with the development team  i.e. that issue is not clearly mentioned in the SRS.

3. Testing may not be done completely.

If the defect is identified with in 3 to 6 months(Project & company dependent) i.e. free support, then we will fix that defect at free of cost.

If the defect is identified after this duration then the vendor will charge with some standard of cost otherwise the vendor will record that defect & will rectify that defect in the next version.

See in this case it depends upon the environment in which he's working like for Ex the latest Active X controls which change every day can be a prob its called as Featuritis.

If the Environment is proper and u didn't find the defect then its ur mistake itself and ur responsible that u didn't raise the defect

The above answers are good,

Also we have to remember that 100% bug free product cannot be assured or submitted to client.

If any major critical bugs are found by the client with Functionality or the Business Rules then naturally tester is to be blamed.

Because as a tester it is the duty of the tester to clearify any douts he gets regarding the business rules and requirements. Based upon the requirements and the business rules the test cases are designed and executed.

Over all if the defect is found by the client then as a tester we have to see that at what Scenario or at what test case we have missed that particular defect and what is the impact of that defect.
