How to prepare a functional requirement document....?

Can anyone brief about how prepare functional requirement documents and which document is required to prepare FRD...?

For writing functional specification, user must have User requirements or business requirement document. UR or BRDs have always contained high level information for instance "application effective date would be January/1/2015. In functional requirement, user needs to define small nitty-gritty or low level details like “New system process is effective with applications received on or after January/1/2015. System cannot process any application with respect to XXX system before January/1/2015. In functional specification, user need to keep in mind one thing that user are communication with Dev and QA and telling them all the information which he/she got during requirement gathering with Subject Matter Expert (SME) or Line of Business.

Functional document must comprise certain points which shows the user requirement in terms of use cases and test scenarios. Functional requirement must cater uses all whim. Interfaces and their validations(non functional) requirement must be part of document.

In order to prepare FRD, you need BRD which contains the high level requirements that were given by business owner. FRD should consists of all the functional requirements in the low level which should also need to include usecases.
