Defect creation and verification can be managed through the defect life cycle management tools like HP QC, JIRA, IBM rational, or other ALM tools which has facility to upload test case and to map the test case with respective requirements.

Ff any test case fails tester can assigned TC/QC defect to respective team to fix (mostly to developer ) once developer fix it he/she will assigned back to respective tester for validation once defect is fixed defect can be closed/reopen if required based on the scenarios.
