This calls for strengthening of 2 different processes:

Change Management process needs to be strictly followed if the requirements are added frequently. As part of Change Management you should stress on:
- base lining the requirement
- Analyzing the new requirements for its impact
- Associating Cost & Effort to added requirements
- Formal signoff of requirement document by client after every change

Review Process needs to be religiously followed to identify the missing requirements. You can have
- Peers reviewing the Requirement document before signoff
- various stakeholder reviewing the requirements’ before formal signoff

Apart from what Poonam had mentioned. I want to add that one should constantly update/review Bi-directional Matrix where we can identify the components that were promised to be delivered to the components that were delivered
