In order to get the Designer field, you should do the following steps..

1. Click on Tools Menu on Top right of QC
2. Click on Customize in the menu
3. QC Project Customization screen will open up.
4. Click on Customize Project Entities Link
5. Expand on Tests
6. In User Field Add New Field (Designer)
7. Save the Changes

Actually you cannot modify System fields just like that. You need to go to script editor in workflows which is complex.

But In User fields, You can add a new field named designer and populate it with values of the user list, so like this you can get a designer field with user values.

If in case designer field is hidden, that can be taken care only by site administration.

This is what I know..not sure if this helps!
