Hi.., All the companies does not follows the same format. Every comapny will have their own formats.Test Case is a Combination of input and Expected out put Format: Test Case No. Test case Type ( +,-) Input Expected Result Actual Result1.1.1 + 365 Success1.1.1-----> whcih refers to Module number, scenario number, testcase number.I think u understood...now...Regards...............Naren...

The main columns for test case are as follows:

1. Test Case No.
2. Test Case Description.
3. Expected Result.
4. Actual Result. 
5. Remark(Optional)

In Quality Center or Test Director, we have many more columns that we can add and maintain as per the Client's requirrement.

For the test case following are the important columns.
The following will be the header part for the test case document.
   - Screen/Function name
   - Screen/Function ID
   - Version/Build number
   - Author
   - Date Created
   - Last Updated
   - Application
   - Component Type
   - Reference Specification Document
   - Reviewed By and Date
The following are the Detail part for the test case document
   - Test Case / Test Step
   - Test ID
   - Test Purpose
   - Test Step Action
   - Input
   - Expected Result
   - Test Result
