The different types of documents prepared by the QA group in order are Test Plan,Test Cases and Conditions and different status reports
Business Analyst prepare requirement documents,Use Cases,supplemental specifications discussing with the business
Developers create data flow diagrams etc.

1. Business Analyst will prepare Requirement doc.

(From the req.doc design doc, use case are prepared)
2. Test strategy prepared by Test Manager
3. Test plan prepared by Test Lead
4. Test cases prepared by testers (included in the test plan)
5. Defect report  are prepared(included in the test plan)

Within QA, usually the QA lead (or mgr) creates a QA plan first which includes (but not limited to):
-Release name & version
-the scope of the release
-description of "hand over" to test group
-needs of the testers (i.e. applications, OS/Browsers, tools, access, software, hardware)
-List of supported systems (browsers, OS, versions)
-general overall description of tests
-any concerns or blocks that need to be resolved prior to testing (i.e time constraints vs. scope; h/w or s/w needed for some tests not available)
-Signatures required for acceptance of the document
-Could also name assignments to particular people/areas (testers and developers)
-reference development docs used to define features and create the tests.

in the initial phase business analyst prepare BUSINESS REQUIREMENT DOCUMENT(BDD), in the analyzing phase system analyst prepare SOFTWARE REQUIREMENT SPECIFICATION (SRS), IN designing phase technical lead will prepare the TECHNICAL DESIGN DOCUMENT (TDD), in the testing phase testers will prepare test case and use case documents, and in delivery maintenance phase testing team will prepare SOFTWARE DELIVERY NOTE (SDN)

The mother of all documents is the business requirements document prepared by BA or client. Based on this BA writes the requirements document.

Based on this, dev team will prepare dev design document. DBA will prepare ER diagram (ERD)
QA Manager will prepare test strategy document
QA lead prepares test plan
QA lead or QA will prepare test data document. Some companies want to be meticulous so they document everything they do.
QA will prepare defect report, write test cases, test progress report. Test progress report can also be prepared by test lead depends on the company.

BA prepares requirements traceability matrix after QA team completed writing test cases.
For each build release notes is published to the entire project team. This one is prepared either by dev lead or build manager (again depends on the company setting).
BA with the help of QA team prepared UAT document and hands it over to business users for UAT.
Before project goes live, QA lead or BA prepare user guide. This ask comes from client.
Learnings document is prepared by dev and QA teams separately after project goes live.
