Load generators will be on host machine, It did not matter where? It matters how we are distrubuting loads.

Installing generators at customer and call center locations will help to log the issues related to network...

Generally main loadgenerators (the ones you use for to generator large amounts of load) should be located as close to the target servers as possible e.g. in the same switch if possble. If your system is small it's not that important where they are. You can also add additional loadgenerators to where the users are and to other locations where you want to have data.

When running the test you should use the closest load generators to run the majority of the vusers and run the far away loadgenerators for smaller amounts. This helps to get an idea how latency, packet loss etc affects your application.

Partially C) If you placed it at the server location you would want to ensure when you setup the Loadrunner scenario  it simulates the latency from the Server to the client sites. (this is a feature which is available in LR9 onwards).  It allows LR to simulate as if it was sending the injection over the network and as a result it would allow you to carry out a more realistic test, and the reponse times you gather would include the networkdelay which the end users would encounter. If you have a look on the controller i'm sure you would find the feature i have mentioned if not, let me know and I will take a look.
