see before starting the actual testing of the application we use to submit the testcase for reviewing purpose. once the application is ready we start testing and if defects are found we log the defect and send it to deveploment team. we can use defect tracking tool to send the defect report which itself includes testcase name, severity, pritority, status, test data,tesr environment, and can attach screen shots and so on..........

swapna answer is more accurate...............Actually the question is not clear in the sense, who have to submit the reports?.....whether the Tester or team lead?Because reports will different for both persons.

As a tester after completion of execution on each build I prepare Status Report
Give report what I did since last build or last week

Defect Report - all defects found till now.

Defect Log- information regarding the single defect.
