The answer to this question depends on whether you want to know the literal or figurative meaning of the term.  Poka Yoke was originally called 'baka yoke' which literally means 'fool-proofing' or 'idiot-proofing'.  This term was later changed by the Japanese to the less offensive 'poke yoke' which means 'mistake-proofing'.   I am going to assume you want the figurative meanings, so I will choose d) both a & b.
