Click on 'Test Plan' tab,
Select the Folder where you want to add the Test case,
Right click on it,
on the popup window Click on 'New Test' button,
then it will pormpts for the test name,
after entering the name click OK button,
the created test will be added to the selected folder.

 As of my knowledge in TD5.0 simply add test cases to  Designsteps tab in Plantests  tab...Let me know if it is wrong answer.
