When you launch your QTP, you will find an option called "add-in-manager" there you can load your add-in.

Add-in can be selected when ever you start QTP, if that is not available you can go to File-->Setting-->Properties-->click on modify you can modify it but it will come to effect once if you restart your QTP

In menu go to
1.Option
2.In genral tab
3.Check the 1st one which is display the add-in manager on start-up.
