Traceability matrix is tracing your requirements to your test cases or any other documents. For eg: you can trace your BRD to your scope/vision document to your use cases to your test cases. Any changes in any one of these would automatically make chages to the rest. You can have any number of fields in rows & columns but two fields that are mandatory are requirements and test cases.

Actually traceability matrix was created for keeping track of Functional Requirement against each Business Requirement. each business requirement will have multiple functional reuqirement mentioning the features, process, outcome and so on. This is part of RUP standard.
