A moderator is a person who mediates between people in a Review Meeting.
A moderator responsibilites include -
1) Deciding the Agenda of the review meeting and ensuring that the meeting stays on the decided lines
2) Decide the duration of the review meeting to ensure that it does not become a tiring process with very less effectiveness.
3) Delegate tasks to Recorder for documentation purpose as well as for recording Minutes of meetings.
4) Mediate incase of an issue during the course of review meeting
5) Final review of the documents that are an outcome of the Review meeting.

Umesh has almost described what are all the roles of a moderator, and in addition to that a Moderator in a review meeting is mostly the Team Leader or the Test Manager.
