Resume should reflect your personality.
No one person knows about yourself. You should analyze yourself and prepare.
There is no use in getting a format and changing the name alone.

Hey...actually its very easy to get a sample resume from google. Just type in the keywords and say resume, you will get a bunch of resumes with nice formats which you can use.

Ideally, the resume title should tell the keywords of the job profile you are interested in.
Start with your full name, contact details and address.
Summarize your background and years of experience.
Give details about your professional background and projects undertaken.
Just dont forget to highlight the keywords in your project.
End up with your personal information like DOB, Permanent address etc.
