    Defect injection rate refers to the number of defects that were discovered and reported during a particular iteration of Software Development.It is the total number of known defects no matter whether they were discovered and immediately fixed or not

    Defect leakage occurs at the Customer or the End user side after the application delivery. that means after the application release to the Client then they start using that application, if they got any type of defect then that is called as Defect Leackage

    Defect Injection is also known as Defect Injuction/Fault Injection/Error seeding/Defect seeding/Mutation testing .
    Error seeding acts as a reliability measure for the release of the product.Usually one group of members in the project injects the defects while an other group tests to remove them.The purpose of this exercise is while finding the known seeded defects the unseeded/unearthed defects may also be uncovered.Defects that are seeded are similar to real defects.Therefor.they are not very obvious and easy to detect.The Idea is while detecting the known bugs unknown bugs might be detected. The goal is to determine the Bug Detection Rate.

    Defect seeding may act as a guide to check the efficiency of the inspection or testing process.It serves as a confidence measure to know the percentage of defect removal rates.It acts as a measure to estimate the number of defects yet to be discovered in the system.This metrics is to understand the quality of a product and would be used to determine "defect review effectiveness".

    Defect Leakage can occur in different phases of the software life cycle development. It could be during the requirement coverage,Implementaion phase etc. It is not possible to iliminate the defect leakage compleately however you can minimise it by adopting the best practices of the software development.

    Most of the time it is impossible to uncover all defects present in software. Wharever defects remain in software and are caught during later stage of SDLC are collated with defect leakage factor. You can track defect leakage in a standard matrix which can help in the further analysis.

    There are some of some of the statistics that can be used to calculate the defect leakage

    Number of defects slipped (reported from production) v/s number of defects reported during execution.

    Number of Defects Slipped / (Number of Defects Raised - Number of Defects Withdrawn)

    Example: Customer filed defects are 21, total defect found while testing are 267, total number of invalid defects are 17

    So, Slippage Ratio is
    [21/(267-17) ] X 100 = 8.4%
