Writing a test plan template for database server conversions

Does anyone have some suggestions on how to write a test plan template for database conversions without being too general? I have little to go by, except that multiple applications on different types of db servers are to be converted to newer db servers of different types and each applications will also be recoded in a new language. I have limited to no access to some existing db servers and applications, and current versions do not exist in a dev or test environment. Seems nearly impossible, if you ask me.
