How can I create web part with user input box

I have uploaded a huge XLS file in the a folder of SharePoint Server2007 site.
How i create a web part with a user input box (either on the web part of as a pop up) that when somebody clicks on it, they can enter a customer name and all the records are then showcased in the web part.
