    Seems you are confused to define a bug you need bug log report not test cases.

    Basically one should consider the following steps during the defect log,

    1. Defect Summary
    2. Defect Description
    a. Pre condition
    b. Steps to reproduce the scenario.

    1.Requiremnt ID(optional)
    2.Testcase ID
    3.Test Description
    4.Test Steps
    5.Expected Result
    6.Actual Result
    7.Status(pass/fail)

    Pre Condition
    Action Items / Input data / Steps
    Expected Results / Pass Criteria
    Test Data
    Pass/Fail
    Defect Summary/Step Failed
    DefectCount_Cycle1
    DefectCount_Cycle2
    Detected By
    Severity
    Status
    Priority
    Module Name
    Developer Comments
    Tester Comments

    It depends on the organization whether want to utlise the same Test case sheet to report bugs or want to have seperate bug reporting sheet. However, it's always easy to track a bug against test case. In that case, below format can be used (we use below format):
    1. Sl.No
    2. Test Case Title
    3. Test case Steps (Provide preconditions under this as all test cases not require precondition data/steps)
    4. Expected Result
    5. Test Data
    6. Actual Result
    7. Test Case Execution Status (Under this, 3 columns: Cycle 1; Cycle 2 and Cycle 3)
    8. Reference/Bug ID (If you have any bug reporting tool, then can give Bug ID)
    9. Priority
    10. Est. Time (This can be used when you are executing Database query related cases)
    11. Tested By
    12. Date
    13. Comments

    To report a bug QA should provide all the necessary data to understand and reproduce the issue by anyone hence QA should provide above all information.

    * Prerequisites
    * Procedures
    * Information if required
    * Tester input/record
    * Verify step
