When and how do you decide to enter a bug report?

After appling correct test on s/w build with correct test data in correct environment also, if test fails, then i will go for bug reporting using Defect Report IEEE 829 format to DTT(Defect Tracking Team).

If the following criteria are met, I will file a bug report:

1) If a defect is found that compromises the functionality of the program under test.

2) If this defect is found using proper test data input.

3) If this defect is found using the correct sequence of test steps.

4) If this defect is found in the proper test environment with all known dependencies applied.

5) If this defect has not already been identified as a known issue that the developers are currently fixing, especially if this fix is targeted for the next build.  In this case, I would simply note to check the build for the fix.  Of course, this process varies between test teams and may not apply to all.

6) If this defect has not already been discovered and entered into the defect management system.  That is to say, if the defect is not a duplicate.
