Following Components to Create Test Plan Document.

a) TestPlan ID
b) Introduction
c) Testing Approach
d) Features to be tested
e) Features not to be tested
f) Entry Criteria
g) Suspension Criteria
h) Exit Criteria
i) Staff and Training Needs
j) Test Deliverables
k) Risk & Mitigations

Please follow the above components. I am sure that you will be able to prepare Good test plan.
