You can use BugZilla Mercury Test Director Rational Clear Quest or a excel sheet to maintain the history of bug filed/introduced till date.

I don't think so.As i know Bugzilla is most popular but it is difficult to installed and work on windows OS.It's Best for UNIX / Linux OS.

U can use Bugzilla.. but it's not an user friendly application...

Quality center is not a free toolll
