  In the defect report we have to mention the following elements:

   Summary (abt the defect),To whom u assigned the defect,In which environment u got the defect,In which testcase u got the defect and expected and actual results.

The key elements in Defect Report and Test case format are the following:

They are

Test case Id, Defect id.

PASS or Fail.

The elements in Defect Report would be Testcase id , Severity, Priority, Module, Platform the application was tested on, Synopsis, Steps to reproduce the bug & finally the reporter of the bug and the developer to whom the bug is assigned The elements in a test case would be Testcase id , Requirement id, Description of testcase, Expected and actual results (i.e pass / fail)
