Mapping the requirements against the test case is called as Tracibility Matrix. It means in order to tell the client that we have met all the requirements as specified in the SRS.

It includes descriptions like: Project name, Date, Change Control, Revision Date,Author,Sections,Summary of changes.

The answer to this question depends on the comprehensiveness of your intended tracking.  The components of a traceability matrix are:

1) Document
2) Version of the document
3) Target identifier
    a) requirement number
    b) specification identifier
    c) any means to isolate and identify source information
4) Project
5) Functional area
6) Test case number
7) Explicit / implicit  (ex: test case may be derived from implicit requirements based on GUI standards)
